Narcissistic personality disorder (NPD) is a recognised condition in the DSM-5 characterised by a pervasive pattern of grandiosity, an intense need for admiration, and a lack of empathy, present across multiple areas of life and causing significant distress or difficulty in functioning. NPD is distinct from ordinary confidence, self-focus, or high standards: it involves an inflexible and long-standing pattern that affects how a person relates to others and to themselves, often in ways the person does not fully recognise. It is frequently accompanied by fragility beneath the surface, a vulnerability to shame or criticism that the grandiose presentation conceals. NPD is one of the less commonly diagnosed personality disorders and requires careful assessment by a qualified clinician.
NPD is characterised by patterns that are present across situations and relationships rather than in specific contexts.

- Preoccupation with fantasies of unlimited success, power, brilliance, or ideal relationships

NPD is best treated with longer-term psychological approaches that work with deep-rooted patterns rather than surface behaviours.
- Schema therapy Schema therapy identifies the early maladaptive schemas underlying NPD, including schemas around entitlement, defectiveness, and emotional deprivation, and works to change them through a combination of cognitive, experiential, and relational techniques.
- Psychodynamic therapy Psychodynamic approaches explore the developmental and relational origins of narcissistic patterns, including the role of early experiences in shaping the grandiose self-presentation and the vulnerability it protects.
- DBT (Dialectical Behaviour Therapy) DBT can help with the intense emotional reactivity, particularly shame and rage, that often underlies NPD, building skills in distress tolerance and interpersonal effectiveness.
